By convention, all Moodle activity tables start with the name of the activity. So for questionnaire, tables would be names "[prefix_]questionnaire[_extra]". If you look at your database, I think it will be self-explanatory. All response data is in tables that start with "[prefix_]questionnaire_resp[onse][_extra]".
Hi. First, great plugin! We're trying to report on responses. Can you assist in where the questionnaire responses are stored in the database? More specifically, what table?
Hi Mike! We will try to install Questionnaire 3.4 to see if that works. I think what my vendor is saying is that Moodle 3.4 is what we need in order for the Questionnaire plugin to work? Unfortunately, Totara 12 is solely based on Moodle 3.3 and they are working on upgrading Totara to work with Moodle 3.4 or 3.5. I will update you if Questionnaire plugin 3.4.4 works or not though!
Why don't you use the 3.4 version of questionnaire? https://moodle.org/plugins/pluginversion.php?id=18486 It will work on Moodle 3.3. If Totara 12 is built off of Moodle 3.3 then it should work fine. Unfortunately, Totara does not make its code available to the public, so I have no way of testing that.
Good morning. My company just upgraded to Totara 12 which operates off the Moodle 3.3 build. My vendor downloaded Questionnaire 3.3.3 and we get errors whenever we try to submit an eval, view responses, or even view data/analysis. My vendor emailed us this:
"mod_questionnaire is using a reserved work “rank” in mysql 8, which is fixed for the 3.4 version of Moodle. I can’t apply the patch on the mod_questionnaire for the current version of Moodle. In short Questionnaire does not currently work in Totara 12. You can petition the contributor to try and get a fix for version 3.3 on the plugin page"
Would you happen to have a patch for this? We've been using the questionnaire plugin for 5 years and all of a sudden, we've lost all access to our evaluations due to a simple upgrade. Any help or advice would be greatly appreciated, thanks!

Is questionnaire not be supported anymore in moodle 3.5 release ? Because i have a bug:
The email notification (with submitted answers) does not support the translation. So it send all the translation tags like "span class multilang" and the "mlang" tags too instead of making the translation

Is it possible to export responses into PDF or any format so the graphic results are still visible? Printing the page as a PDF doesn't print the entire response.
Exporting as CSV is possible but all the nice graphs get lost.
I am using Moodle 3.5.9 and the Questionnaire version 3.5.4 (Build - 2019032100) 2018050109
