In the Admin>Application Form>Edit this plugin page, the field is called 'Bug tracker URL'. ie: https://moodle.org/plugins/edit.php?plugin=mod_apply
You do not necessarily need to create your own system. If you do not have a preferred one, we're always happy to create a project component to track issues in tracker.moodle.org, see link: https://tracker.moodle.org/browse/CONTRIB/component/10142
If i understand you right, you're thinking of using the forum as an issue tracker? It may not be as scalable but it is your choice, you can always change it in future

Thank you for sharing this. This installs fine on 2.4 and seems to work well .
Have you spoken to Andreas? it would be constructive if he knew about this in case an effort can be made to combine the best of both these modules into feedback. ( as feedback into feedback module )
Before approving this, could you please consider and let us know about adding a Issue tracker URL, to go to when users would like to report an issue.
