I installed this latest version this morning onto a fresh Moodle 2.6 installation and this error came up when I tried to add quizzes.
Coding error detected, it must be fixed by a programmer: moodle_database::insert_record_raw() no fields found.
All the other settings are correct, such as admin username and password, as we have been using this module on Moodle 2.5 for the last six months.
I see the source URL. Please add documentation URL when possible. Reapproving this.

Hi Aparup,
thanks for the feedback on the Reader activity module. I have removed the superfluous lang files. Could you possibly add record in the tracker for new issues regarding the Reader module that are automatically assigned to me. Or should I ask Anthony B. to help me get that set up?

i got this validation warning during installing this plugin (into 2.6 beta) via
the install button: 'Multiple English language files found' , do note that
translations are provided in amos. you might want to remove the other
reader.*.php ones.
Could you add a issue tracker url ?
